If your ballot was lost or discarded, you may request a replacement ballot. Enter the information below, then click Submit. If you already called or emailed Pro Elections for a replacement ballot, do not also submit a request here. Please note that ballots are irrevocable once received by the Inspector of Elections, so a replacement ballot will not be accepted if you have already submitted your ballot. Please allow up to 7 days for delivery by the US Postal Service.
